[ Naegi's been going downstairs several times every day to check up on Ishimaru, to see if anything's changed. And after finding out that falling into a coma means going home, he's been thinking about the state Ishimaru's going to be when he comes back. Everything spiraled into hell after the second trial and Naegi's attempt to make it better back then only resulted in Ishida. Which made Ishimaru less of a shell but it didn't exactly fix anything.
When he comes in and sees Ishimaru, the initial relief seems to fade when he notices that disconnected look in his eyes and realizes what it means. It means he should go back upstairs and get rid of the butter in the kitchen. ]
